Which doctor should I go to for a bloodied toenail? (Tokyo)

So, a few months ago, I dropped a bathroom spray can on my big toe, and it bled internally on the nails, but it didn’t hurt. Just a few days ago, it started hurting so much that I decided to search for a clinic/hospital. I just hoped that the nailbed would just pop off, so I painted my nails to hide it, but because I wanted to search for a hospital, I decided to remove the nailpolish, but it hurt so bad to put pressure that I only do it as much as I can.

I searched for a foot specialist, but when I read the description, the only thing about nail that they’d do is about ingrown toenails.

I thought it was an ingrown toenail, but when I dug into it, there’s nothing but a white discharge. I’m thinking at this point, pus?

So I searched for a 外科 because it is a wound, is it not? All the ones downtown are just about surgeries in the description. And by this point, I just wanted to get rid of my toenail at this point.

So I went to my local general hospital, and oh my god. I normally don’t mind 1star reviews on google maps because whenever I do go to a clinic with 1 star, it’s just hilarious when expectation and results are different.

When I went into the hospital, I asked the front desk, a 総合受付, and wrote on my form for a 外科. Convo went like this

FD: Since you just drop something on the toenail, it is not a scratch or a cut, so you probably can’t go to a 外科. Let’s try a 整形外科 (orthopedist)
Me: Why would I want an orthopedist? I said it’s just my toe. I don’t have any broken bones.

I argued my point multiple times saying I don’t need a scan for broken bones. I just want the toenail gone. The FD also didn’t know and then said probably go to a derm bc it’s effecting the skin. So I asked and argued because it was confusing and the FD wasn’t really clarifying.

And then the FD said, ‘okay, so the departments for 外科 and 整形外科 are the same, so ask the nurse about it. Otherwise, you can just switch’. Fair enough.

So I went and got my appointment, and I was called in. It was a major disappointment.

The doctor said it was just an ingrown toenail and since it’s not infected (because, as you can see, the side that hurts is not bulging and thus no infection. I just had to wash and be careful of the toe.) She said something like ‘just grow out the toenail to the sides, and you should be fine. Don’t cut the sides too short’. She said that because I dropped a heavy object on my toe, it causes the toenail to curl up (?) causing issues to it.

It HURTS, and I wanted to prevent something before the infection came because once more, I emphasised on the fact that there was *a white discharge*.

Not infection. She just looked at it, barely inspected it, and sent me off, recommending me to a derm to take a look at it if I want.

That derm was already closed a long time ago. The elevator to go there wasn’t even functioning.

They really deserved the 0 star many people were saying on GM.

I’m at my wits end, it hurts and I can’t even cut off the toenail without feeling pain. WHAT CLINIC SHOULD I GO TO!??!?!

  1. Heat up a needle on the stove and pierce the nail to relieve the pressure.

    Grab a beer, kick up and watch a movie.

  2. Go to a major (as in university) hospital, not a clinic. You may have to pay a non-introduction fee, but you’ll get the proper treatment – which means antibiotics.

    In the meantime, soak the infected foot in warm water 15-20 minutes a few times a day. This helps pus to drain from under the nail and relieve the pressure.

    Don’t pierce the nail yourself – that can do more harm than good.

  3. Okay that is insane. I went to a hospital and saw a dermatology doctor (hifuka) because my toenail had become really painful. It’s in Osaka [but this is the place – Sumitomo Hospital](https://www.sumitomo-hp.or.jp/about/shinryoka/hifuka.html), maybe the website will be helpful. It’s a big hospital, not a clinic

    I walked in, she had me lie down on the bed, looked at my toe and realised a triangle shaped bit of nail was sticking in the side causing pain. Not at all infected, just very painful. Within a few minutes she’d given me an local anaesthesia injection and was cutting the bit out. The injection was the worst pain of my life but cutting it was the best feeling ever 😌 she was so awesome

    Anyway. That is the type of thing I’d expect you should be able to get, at least getting them to look properly and even maybe remove the painful part if needed. Try going to a hifuka in a big hospital! Good luck!

  5. You can use a cuticle cutter to remove skin on the side of toenail. Instant relief. You can also buy a wire on Amazon that will straighten it out
