Link to Visa Form – Change the status of residence – for printing?

Agh! I need to file a “Application for Change of Status of Residence.” I am currently INSTRUCTOR and need to changeover to SPECIALIST IN HUMANITIES / INTERNATIONAL SERVICES.

I have found other forms like initial visa, initial app for humanities, initial app for instructor, etc – but for whatever reason the specific form to change status and change status to humanities is alluding me. I may be imagining this, but I think the “change of status” applications are slightly different based on what status you are going for, so a “change status….” to Instructor is a wee bit different than “change status to…” Humanities/International Services.

I do need the PDF – paper form, as my employer wants the paper form for the employer info. They like it old-school in that manner.

If some kind soul can link me to the forms I need. it is much appreciated.

ps: I can’t do this changeover online online as my “my number” is still paper receipt, not the plastic card.
