Where can I find infos in English on how to fill in the aoiroshinkoku?

I want to launch my own little business (in addition to my part-time job where I receive a salary, so salary + jieigyo).
The tax office gave me a green and white document (what I will have earned and spent) and told me to come back with it at the end of the year. I have not the slightest idea on how to fill it in…
So if you know a well-done site (for dummies 😛 ) translating the document and explaining how to, it would be really helpful!
