I am a permanent resident 永住許可 I am planning to return to the US permanently and am wondering what I need to do about canceling my health insurance and city registration. I already found a post that talks about getting a 5 year re-entry permit. I plan to do that.

There are lots of things you need to do at city hall / ward office. You should go and tell them you’re there for tenshutsu todoke (moving-out notice) and they should be able to tell you what else you need to do there
Could you share the post about the 5 year re-entry permit, please? The application form seems to have a “planned return date” that I’m not sure how we should fill out.
