I’m not familiar with that particular drug, but I’m gonna hazard a guess and say proper medical subsidies.
There’s a variety of reasons generally applicable.
Different pharma companies licensing the drug in different countries, government setting price caps differently, market demands etc.
Take your pick.
Although I’ll disagree with the last part of your post, from what I’ve seen most drugs are significantly more expensive in the US because private insurance is footing the bill and not the government.
There aren’t a lot of overweight people to gouge for the weigh loss benefits. It’s probably just prescribed here as it should be for diabetes

Used to work for a pharma company and it is a very common subject how much we try to overcharge vs other countries as there are price regulations

Epipens are also like 60 bucks here, not 600 or more.
Because everything is cheaper outside US.
Actually, due to weak yen, everything is much cheaper in Japan than in the US now.
As for Ozempic, they say it’s in high demand in the US while it’s not in Japan. It makes even bigger difference in its prices between Japan and the US.
Japan has price controls on medicine, US doesn’t.
Most drugs in the USA are significantly more expensive than the drug sold abroad
