Go to your regular doctor and if they can’t do it they can give you a referral to a hospital. This is how it works in Japan, much faster and cheaper if you do have to go to a bigger hospital this way.
Not infected, but I’d had a cyst on my scalp for years. Finally decided to get it removed, went to my local dermatologist who gave me a referral to a larger hospital. It’s my understanding that you can go directly to the hospital if you want, but it ends up being more expensive without a referral.
One visit for bloodwork, imaging, all that pre-surgery goodness. Next visit for surgery in the early morning, took maybe all of 30-45 minutes on the table. Another visit some weeks later to have the stitches removed.
All in all, a good experience – minus the patch of hair they rightly had to shave on my head
Not in Tokyo, so can’t give you a specific recommendation, but some bigger dermatologists will do it. Or plastic surgeons. There are also specialists who do nothing but cyst removal. You can search for 粉瘤(ふんりゅう) 手術.
Had one on my back that had been there for over 8 years removed at a random chain dermatologist. Was easy and quick. It’s called “Clinic For” and there are quite a few of them.

Yes. I went to my local dermatology clinic and told the doctor that a dormant cyst started itching. He then removed it / drained it in situ with local anesthesia. I was probably in the clinic for a maximum of 5 minutes and paid like ~5000yen.
I had a similar situation two years ago and Meguro Dermatology handled it all. They have an English speaking doctor (if that helps), and everything was handled quickly. They are a few minutes walk from Meguro Station.
