Statement of Consent Notary Apostille

Has anyone needed an apostille when getting a notary outside of the US embassy at a Japanese Notary Public?

The document will be used at the US embassy. The embassy website only says a notary is required. But I heard I may need to get an apostille from the ministry of foreign affairs. I can’t find an answer anywhere.
After contacting the US Embassy, they still could not give me a clear answer. Only that I may possibly need it.

The document is a consent form. I’m reporting a birth, and my wife cannnot attend. She has to sign and notarize a DS-3053.
On the embassy website under the link to the DS-3053 it says,
“In Japan, a DS-3053 notarized by a Japanese Notary Public is acceptable.”
It does not mention an apostille. So, do I need one?
