Advice for a handicapped Traveler

My husband and I will spend three weeks this November in Tokyo, Kanazawa, Kyoto and Naoshima Island. We are slow travelers to begin with, but now that I need a hip replacement (scheduled after our return) we are suddenly slower. I worry about catching trains (with luggage) and generally getting around and finding places to rest. We have a moderately ambitious schedule that we can amend, but advice from any one who has had to navigate with a slow gait and a cane in any of these places would be exceedingly welcome. I can send a detailed itinerary if that would help.
