If you were able to get it from a doctor in Tokyo in the past, couldn’t you ask him to write a letter in Japanese for a future doctor? The only problem would be the language-barrier I presume? So, the letter would eliminate that problem I suppose. Having been prescribed Concerta here for over 2 years, conversation never goes any further then “so, any negative symptoms”? for which you don’t need to be fluent in Japanese for. Otherwise, there are some websites that show English speaking doctors in your vicinity but I forgot it’s name. Chance of an English speaking one outside Tokyo are relatively small but not impossible I’d say.
