Usually it’s most broad to specific.

Family name Given name
Doesn’t matter. You can even write it the same as you would in English and it’ll arrive fine. When my family sends me mail they just write my address the American way and it gets here fine
A convention here is to write the family name all in capitals when writing in English. Makes it very easy to tell. John DOE. Maybe give that a go if in English
