Girlfriend and I will be traveling from February 1-23 here is our brief itinerary which is missing a couple days. Any advice or input please let me know. I used to live in Japan this is my girlfriends first time.

I would think it would make more sense to do Tokyo to Mt. Fuji to Hakone to Kyoto vs. Tokyo to Hakone to Mt. Fuji to Kyoto. The trip from Tokyo to Mt. Fuji is not so hard, but the trip from Mt. Fuji to Kyoto is more challenging since you need to take a bus from Kawaguchiko to Mishima (and the bus only runs so frequently.)
It’s probably cheaper to just pay out of pocket for all your tickets since I don’t think a pass would pay off and yeah I would definitely fly back from Nagasaki to Tokyo unless you have specific places you want to see on the way back. It’s a long trip otherwise and unless you pay out of pocket for a Nozomi, there are no direct trains between Hakata on Tokyo covered by a JR Pass.
I guess if you wanted you could always break up your trip and do say Hakone to Osaka, spend time in Osaka, go onto Hiroshima, then Nagasaki and then hit Kyoto on the way back before ending in Tokyo if you are truly opposed to flying, but I think flying makes the most sense.
I went to Mito around February 23rd in 2020 for the start of the ume festival or you can day trip to Izu (or overnight in Izu) to see the Kawazu sakura.
