Struggling to find any pay-as-you-go place to do hormones check blood testing in Tokyo (test, free test, prolactin etc. For SBHG – it seems no hope to do it in Japan even in a big hospital) . Can anyone advise anything please?

Go to a men’s clinic (google it). They will do this sort of stuff.
This hospital charges 27,500 yen for urinalysis, general blood tests, LH, FSH, prolactin, free testosterone, zinc level, PSA test, and abdominal ultrasound.
You should go to a urologist. Treatment of male menopause is popular and many hospitals offer insurance coverage for the test.
For tests and treatments not covered by insurance, it is better to find a small clinic that sells them.
