I have question about this certificate.
Do hospitals take them from me for each procedure?
i.e., is this certificate expended everytime I visit the hospital for a procedure and therefore I have to get a new certificate every time?
Or do I just show the certificate and it will be returned it to me usable for next time (or in another hospital)? (similar to the insurance card)

My mom got it in January for a knee operation. The hospital gave her the form to fill out and send. I took her to the ward office to receive the certificate. She shows it every time she visits the hospital for her surgery-related visits, like you would a 健康保険証, and she is charged nothing, because it is treated as part of the same treatment. I think it expires in 6 months. I don’t know if she can get an extension after that.
