I can suggest Dr. Noda, Director at Tokyo Dermatology in Ikebukuro. He also studied in the US and speaks excellent English (if that’s important to you). They take insurance. If you do decide to go there get there early – they get busy *very* quickly.

Most dermatology clinics will do it. Search for 皮膚科 in your area.
Usually the removal is covered by health insurance, but testing for cancer is not so you have to pay extra if you want that.
Are you looking for the laser treatment or cut it off?
The place that did the skin cancer on my face seemed pretty popular among the foreign population – Meguro Dermatology Clinic めぐろ皮膚科クリニック, don’t know if they do the laser treatment because that’s not what I was in for.
