Itinerary Check: First time in Japan with family including 2 teenagers for 16 days

Hi All,

My family has been wanting to go to Japan forever and we are finally ready to book. We are planning the travel from end of Sept to the first 2 weeks of Oct. My teenage kids are a big fan of all things anime and manga as well as lovers of the Japanese cuisine. I would like to visit the classics, get a culture immersion and enjoy the food. We have prepared a tentative itenary and would like advice as below:

* Is the itinerary too packed or doable and does it follow a logical progression keeping in mind travel between the locations?
* Is it recommended to visit both Hakone and Mt/Fuji five lakes area or either one will suffice? If only one, which is better to visit?
* Based on our travel, would a 7 day JR pass be recommended?
